What You Need to Know Under the general supervision of the Director of Clinical Programs: Care Transitions, the Care Continuum Liaison responsible for partnering with and liaising between HH, HOS, PCS and PC branches to achieve optimal patient satisfaction & outcomes by supporting/facilitating successful patient transitions between HH, HOS, Palliative Care and PCS in shared areas. This position directly supports the organizational/financial goals of the Care Continuum.

QualificationsBe the Best Care Continuum Liaison You Can Be
If you meet these qualifications, we want to meet you!
- 1- 3 years' experience in Hospice and/or HH Care
- Current RN or LMSW licensure
- CHPN (certified Hospice and Palliative Nurse) certification a plus but not required.
- Experience and basic understanding of HH OASIS, and Hospice eligibility
