You'll assist and counsel as requested, on personal, financial, and work-related difficulties, and advocate for Partners and their families to connect them to community resources and assistance programs.Once you're eligible, you'll become an Owner in the company, so we're looking for commitment, hard work, and focus on quality and Customer service. 'Partner-owned' means our most important resources--People--drive the innovation, growth, and success that make H-E-B The Greatest Omnichannel Retailing Company.Do you have a:HEART FOR PEOPLE... advanced interpersonal and customer service skills?HEAD FOR BUSINESS... ability to apply knowledge / resourcefulness to patient advocacy?PASSION FOR RESULTS... drive to deliver services that set our health care apart?We are looking for:- a degree in social work and a Texas SW licenseWhat is the work?Patient Care:- Provides social work services to patients and their families; coordinates outpatient needs for clinical/non-clinical services / information, community resources, financial assistance programs, and long-term activity of daily living needs outside benefits criteria- Provides limited, short-term counseling and therapy services to patients ex. crisis Intervention, motivational interviewing, strength-based- Facilitates referrals to other services (substance abuse treatment, specialty care, and community resources)- Performs comprehensive assessments; documents findings; monitors / evaluates progress- Assists high-risk patients or those with multiple diagnoses who may benefit from social care coordination, contracts, providers, facilities- Navigates patients with emotional and behavioral problems- Supports patients on managing ongoing health conditions; encourages them to make positive behavior changes- Monitors / documents patient compliance with behavioral health education plans; follows up as appropriate- Provides clinical support to the health care team to establish care plans; coordinates with primary care providers to identify / assess issues and improve health outcomes- Promotes multi-disciplinary collaboration between health care team members- Serves as subject matter expert on short- / long-term disability, benefits, and community resources
- Navigate patients to state-funded health care plans (Medicare, Market, SSID)- Conducts / assists in individual, group sessions for preventive / health maintenance topics- Maintains compliance with state-required professional education requirements; attends education / training offerings to keep up with changes in the fieldWhat is your background?- Bachelor's degree in social work, counseling, psychology, or related field; master's degree in social work (LMSW)- Active Texas SW license- Experience in crisis intervention- 3+ years social work experience in a health care environment- Experience working with electronic health records (preferred)Do you have what it takes to be a fit as a Social Worker at H-E-B?- Knowledge of disease management, managed care, medical terminology, referral / claims processes- Proficiency in PC / software skills (e.g., MS Office and departmental-specific programs)- Advanced interpersonal and customer service skills- Advanced written / verbal communication skills- Critical thinking, analytical, and problem-solving skills- Time-management and organization skills- Ability to manage multiple priorities and shift focus between tasks / projects
