Responsibilities
- Conduct initial patient assessments and develop individualized care plans tailored to patients' health needs.
- Provide skilled nursing care, including wound care, injections, medication management, and monitoring vital signs.
- Educate patients and their families on health management, medications, and preventive care.
- Collaborate with the healthcare team to coordinate care and communicate patient updates and changes.
- Document all patient care activities, assessments, and communications in a clear and timely manner.
- Perform patient evaluations on a regular basis to ensure their ongoing health and recovery.
- Assist patients with activities of daily living as necessary to support their independence.
- Registered Nurse (RN) or licensensed Practical Nurse (LPN) licensed in the state of practice.
- Bachelor's degree in Nursing (BSN) preferred; associate degree acceptable for RNs.
- Previous experience in home health care is highly desirable.
- Strong assessment and clinical skills to evaluate patient needs effectively.
- Excellent communication and interpersonal skills to build rapport with patients and families.
- Knowledge of medical equipment, disease management, and home care regulations.
- Ability to work independently and manage multiple cases effectively.
- Current CPR certification and other relevant certifications preferred.
